Take Control of your Healthcare! Download The Barrow Patient Resource Guide November 2, 2012

Posted by mvarlan in Uncategorized.

The Barrow Patient Resource Guide is designed to help you take an active role in your care by keeping your medical information organized in one centralized location and communicating your health information to your healthcare providers clearly, efficiently, and accurately. I can also help you make the most of your doctor’s appointments.

Filling out the forms in your Patient Resource Guide may take some time initially, but it will save you time in the future because you will have all of your medical information in one centralized location. If you do not have all of your health information, just start with what you have. Take your notebook with you to all appointments, procedures and hospital visits.

The more information you can provide for your doctors during your appointment, the better your doctors can understand your healthcare needs, and the more information they can usually give you.
Download the forms from this link below and insert them into a three-ring binder. You have now made the first step towards taking control of your healthcare.
